Any patient on long-term treatment will be given a request slip listing every item they are permitted to have without seeing a doctor.
Requests can be made by ticking the items you require and putting the slip in the repeat prescription box at the reception window or by sending it by post.

Patients registered to book appointments by internet can also request repeat prescriptions online.
Please note that we do not take repeat prescriptions by telephone except for housebound patients - call 020 8250 7530. Please allow 72 hours for your prescription to be processed. We encourage electronic prescribing.
